Description

Root Cause Analysis is often used in Occupational Health and Safety as a lesson learned tool to improve processes and overall approach to hazard control. The process can also be used to analyze issues that may have occurred while operating a Water and Wastewater Facility.

In this workshop, attendees will be introduced to the Root Cause Analysis and processes, interact with some live examples and walk through a Root Cause Analysis of an actual event. They will learn to understand why things occurred and make changes to processes to avoid them in the future.

Topics include:
-The basic principals of Root Cause Analysis
-The processes used when conducting a Root Cause Analysis
-How to conduct a Root Cause Analysis investigation, and
-Examples of actual events in water and wastewater facilities
